The Enforcement Division of the ABC Board is a segment of the Alabama police system. Each ABC enforcement agent is a commissioned peace officer and is provided with all necessary equipment. This form is a record of the equipment that is issued to an agent. When equipment is replaced or transferred this form must be redone or amended. Information available on the form includes agent's personal data (name, employee number, social security number, work and home telephone numbers), description of equipment issued, serial number and state number of equipment, and signature of agent to whom this equipment was issued.
